About Campaspe PCP

Catchment Plans

The Campaspe PCP currently has a three year catchment wide plan for partnership activities.

For 2009-2012 the primary priorities for Campaspe PCP will be the following;

Service Coordination

• Improve inter agency communication and practice
• Increase common tool uptake
• Support uptake of service coordination and care planning practice
• Implement good practice pathways

Diabetes

• Increase self management capacity
• Implement Models of Care
• Increase physical activity participation and access to nutritious foods
• Create supportive environments
• Influence change

Social Connectedness/ Mental Health Promotion

• Increase mental health awareness and literacy
• Support drought affected communities
• Reduce the harm and impact of problem gambling
• Support and encourage social inclusion
